To whom it may concern:

The XYZ collection account is not mine and does not belong to me. Please remove the XYZ collection account from my credit report immediately.

Because of the XYZ collection account on my credit report I have been denied credit and I am not able to apply for credit, the collection account on my credit report may also be harming me in other ways such as insurance applications, etc.

If you believe your records are accurate then please send me with written proof in the form of contracts, receipts or any other documentation that will prove the alleged collection account belongs to me.

Please act quickly on this matter in accordance with the Fair Debt Collection Practices Act as my creditworthiness has been severely affected.

Please only contact me to send proof of the alleged debt one time. Otherwise, do not contact me regarding this matter again.
